UPS shipping labels are still available. To request a shipping label, please email [email protected] and include the following information:

Once your request has been received, we will process it and email your UPS shipping label.
Shipping rates are based on an average package weight of 3.0 lbs.
If your shipment exceeds 3.0 lbs. and incurs additional UPS charges, the additional shipping costs will be billed to your account.

Kit includes a plastic sample bag for packing the sample, a sample submission form to select your analyses, and a pre-addressed postage-paid return envelope. The sample submission process is easy: simply seal your sample in the plastic bag, fill out the sample information sheet, place both in the return envelope, and drop in the mail.
