Equi-Analytical Analysis Kit

Kit includes a plastic sample bag for packing the sample, a sample submission form to select your analyses, and a pre-addressed postage-paid return envelope. The sample submission process is easy: simply seal your sample in the plastic bag, fill out the sample information sheet, place both in the return envelope, and drop in the mail.  Kits are shipped to you free of charge but are charged a fee when processed by the labs. Please indicate on your submission form that you used the mailer as your shipping method and include the $4 fee with your total cost when submitting the sample to the lab.
